All applications for Danish registration (license to practice) are processed and decided by the Danish Patient Safety Authority.
To ensure the validity of your registration (license to practice), all applicants must have their credentials verified by the competent authority in the country of training or their most recent country of residence. There are two ways to accomplish this:

Complete the provided form (in Word format) and submit it directly to the Danish Patient Safety Authority.

Obtain a certificate from the registration authority that includes the same information. The certificate must bear the official letterhead and stamp of the issuing authority.
Please note that only certificates or completed forms sent by your official authority will be accepted. Submissions from applicants or third parties will not be considered.
