Application for authorization for midwifes (EU/EEA)

Use this application form if you have completed your training in an EU/EEA country or Switzerland and you are a citizen of one of these countries.

Your application will contain 4 steps

  1. Completing the application form.
  2. Uploading the documentation.
  3. Approval of the entered information.
  4. Payment of the fee.

We will send you a receipt for both application and payment by Digital Post, if you have a Danish CPR number, or to your registered email address.
