2. By ordinary mail - sign the completed form and send it together
   with the required documents to: Danish Patient Safety Authority, Registration and EU Health Insurance, Islands Brygge 67, 2300 Copenhagen S

 

Please note: The Danish Patient Safety Authority can only process your application when the electronic form and the signed form including the required documentation have been received.
