Verification of degree

All applications for Danish registration (license to practice) are processed and decided by the Danish Patient Safety Authority.

All applicants are required to have their degree verified by the educational institution. Verification can be done in two ways:

1. By completing this form (word) and sending it directly to the Danish Patient Safety Authority.
2. By issuing a certificate containing the same information. The certificate must carry the official letterhead and stamp of the educational institution.

The certificate or completed form must be sent by your authority to the Danish Patient Safety Authority. Certificates/forms sent by the applicant or by another party will not be accepted.
